Please note if your sample(s) is/are received after 12:00 PM US Eastern Time (GMT-5) they will be queued for analysis the FOLLOWING Business day.

NOTE: A sample submission form must accompany any samples shipped to Triclinic Labs and in order for a project to be initiated. No Projects will be initiated without a quote.

If you prefer the manual process, follow these steps:
- Print out Sample Submission Form(s) in MSWord or PDF format (see below)
- (Optional) Request 222 Form for Controlled Substances Only
- Print & Attach Shipping Label
- Include MSDS
- Ship samples with Form(s) & MSDS
