Triclinic’s existing sample-submission guidance is easy: a sample submission form must accompany shipped samples, and projects are not initiated without a quote. Use the web-based system when possible, or use the legacy PDF/Word forms and shipping label when the manual process is needed.

Use this page only when samples are ready to ship and a quote, proposal, or approved pricing is already in place. Each shipment should include the information needed for receiving, chain of custody, safety review, storage, testing, reporting, and sample disposition.
